港区では、在住外国人の皆さんが安心して病院や薬局に自分の症状を伝え、的確な診断・治療が受けられるよう、｢みんなと安心医療手帳」を作成しました。この手帳は、過去の病歴や個人データ欄、全身及び部位別の症状を英語、ハングル、中国語で表示し、日本語と対訳させています。
配布対象は、港区在住外国人。
申込み方法は、窓口受取りの場合は、申込書に必要事項を記入し、各総合支所区民課、保健サービスセンター、区役所３階国際化推進担当でお申し込み下さい。
郵送希望の場合は、みなとコール03-5472－3710へ、お電話でお申し込みいただくか、〒105-8511港区役所国際化推進担当へ申込書を送付して下さい。

Minato City has released the Minato Medical Handbook to help foreign residents in the city communicate their symptoms to hospitals and pharmacies so they can receive the correct diagnosis and treatment. The handbook has a section for information such as medical history and personal data, and alongside the Japanese, provides English, Korean and Chinese translations of whole body symptoms as well as symptoms for specific parts of the body.
Those eligible to receive the handbook are foreign residents in Minato City. To receive it in person, fill in the application form and hand it in to the Residents’ Section at any of the Regional City offices, the Public Health Service Center, or the Global Community Planning Subsection on 3rd floor of Minato City Office.
