港区では、在住外国人の皆さんが安心して病院や薬局に自分の症状を伝え、的確な診断・治療が受けられるよう、｢みんなと安心医療手帳」を作成しました。この手帳は、過去の病歴や個人データ欄、全身及び部位別の症状を英語、ハングル、中国語で表示し、日本語と対訳させています。
配布対象は、港区在住外国人。
申込み方法は、窓口受取りの場合は、申込書に必要事項を記入し、各総合支所区民課、保健サービスセンター、区役所３階国際化推進担当でお申し込み下さい。
郵送希望の場合は、みなとコール03-5472－3710へ、お電話でお申し込みいただくか、〒105-8511港区役所国際化推進担当へ申込書を送付して下さい。

Minato City has released the Minato Medical Handbook to help foreign residents in the city communicate their symptoms to hospitals and pharmacies so they can receive the correct diagnosis and treatment. The handbook has a section for information such as medical history and personal data, and alongside the Japanese, provides English, Korean and Chinese translations of whole body symptoms as well as symptoms for specific parts of the body.
Those eligible to receive the handbook are foreign residents in Minato City. To receive it in person, fill in the application form and hand it in to the Residents’ Section at any of the Regional City offices, the Public Health Service Center, or the Global Community Planning Subsection on 3rd floor of Minato City Office.

In order to further advance the internationalization of the city and enhance the administrative services available to foreign residents, Minato City instituted the draft ‘Minato Internationalization Master Plan’. Minato City has publicly announced the plan and is now asking residents of the city to submit their opinions. Thank you in advance for your cooperation.
Those eligible to submit opinions are people who live, work or go to school in Minato City.
The draft plan can be seen at the Global Community Planning Subsection on the 3rd Floor of Minato City Hall, and on the Minato City website.
Your opinions can be submitted through the Minato City official website or by mailing or faxing them by Tuesday March 2nd, to the Global Community Planning Subsection, located at Minato City Hall, 105-8511. The FAX number is 03-3438-8252.

2010年1月1日現在、港区に居住している人で、昨年収入があった人は、2月16日火曜日から3月15日月曜日までに、まず所得税の確定申告をして下さい。税務署に確定申告をすれば、住民税の申告をしたものとみなされますので、住民税の申告は不要です。また、勤務先から港区へ給与支払報告書の提出があり、この給与所得以外に所得のない人も、申告は不要です。申告書用紙は、税務課課税係および各総合支所で配布します。

People who were residents of Minato City as of January 1st, 2010 and received income in the previous year need to file an income tax return between Tuesday February 16th and Monday March 15th .
If you file an income tax return at the Tax Office, you will be regarded as having already filed Resident’s Tax, so you will not have to file Resident’s Tax separately.Also, if your employer has submitted a withholding statement to Minato City and you had no income from other sources, you are not required to file Resident’s Tax.
Tax return forms are available at the Tax Assessment Subsection of the Taxation Section as well as at each branch of the city office.

② 1997年4月2日から1998年4月1日生まれの中学校新１年生

Minato City’s Board of Education is distributing enrollment notifications to families of children who will enroll in municipal elementary or junior high schools in April 2010. If you have yet to receive this, please ask.
Those eligible are children born between April 2nd 2003 and April 1st 2004 who are enrolling in the 1st grade of elementary school, and
children born between April 2nd 1997 and April 1st 1998 who are enrolling in the first grade of junior high school.
If you are a foreign national with a child that falls into one of these categories and you wish to enroll them in a municipal elementary or junior high school, please contact the School Administrations Support Subsection of the School Affairs Section on 03-3578-2111 (extensions 2726 to 2729).
