2010年4月に港区立小・中学校の入学を希望する外国籍のお子さんの入学申込を、10月1日(木)から港区役所内の教育委員会学務課で受け付けます。
お子さんの住所・生年月日などが確認できるもの(外国人登録証など)をお持ちください。
なお、手続きは日本語で行われますので、通訳が必要な人は日本語のわかる人と一緒にお越しください。
また、港区では学校選択希望制度を導入しており、入学の申し込み手続き後に、「学校選択希望票」等の関係書類を発送しますので早めにお申し込みください。

Applications will be accepted for children with non-Japanese nationality who hope or whose parents hope their children to enter the public elementary or junior high school in Minato City from April of 2010. The School Affairs Section of the School Board in Minato City Office will accept applications beginning on Thursday, October 1st.
Please take an official document with you to confirm the identification of applicable children such as name, address, and date of birth.
It is asked that you be accompanied by a fluent Japanese speaker as the procedure will be conducted in Japanese only.
After your application is accepted, you will receive documents regarding the school selection system used by Minato City. For this reason, earlier application is recommended.
If you have questions or concerns, please contact the School Administration Support Subsection of the School Affairs Section at 3578-2111 ext. 2726 thru. 2729.

定額給付金の手続きがお済みでない人へ港区からのお知らせです。
定額給付金の支給には申請が必要です。申請期限は、受付開始日から６カ月間と定められており、港区における申請受付期間は、4月7日(火)から10月7日(水)までとなっています。
この期間内に申請がない場合には、定額給付金の支給を辞退したものとみなされます。まだ、申請がお済みでない人は、10月7日(水)の申請期限までに申請してください。
また、紛失等により申請書がお手元にない人には再発行しますので、ご連絡ください。

ポリオはポリオ菌によってうつる感染症です。症状は、発熱・嘔吐などですが、麻痺などの後遺症が残る場合もあります。
港区では、ポリオの予防接種を無料で行います。
対象：0～９か月の乳幼児、10～90か月の乳幼児
申し込み：事前予約制です。電話で、みなとコール(℡5472-3710)へ。
当日は、母子手帳と区から送付した書類を記入の上お持ちください。また、日本語のわかる方と一緒にお越しください。

Polio is a serious, infectious viral disease. The symptoms are fever and nausea. In some cases it can cause muscle weakness or permanent paralysis. Minato City provides polio vaccination for free. Babies of nine month old and younger and children between ten and ninety months old are eligible for this vaccination.
To receive the vaccination, you will need to schedule an appointment in advance by calling Minato Call at the following number: 5472-3710.
Please fill out the forms which have been mailed to you from the City Office, and take the forms and your “Mother and Child” handbook with you. It is asked that you are accompanied by someone who can speak and understand Japanese when you go for the vaccination.
You will find more details on the documents you have received from the City Office or in the September issue of Minato Monthly.
